I am an out-of-network provider, which allows me to focus on individualized, high-quality care without the constraints insurance panels can place on treatment. For every session or service, I provide a detailed superbill that you can submit to your insurance company for possible reimbursement, and I’m happy to help you understand your out-of-network benefits and how to work with your insurer to access them.
Questions to Ask Your Insurer
Do I have out-of-network mental health/behavioral health benefits?
What is my deductible, and has it been met?
What percentage of the session fee is reimbursed after the deductible?
Do I need pre-authorization for psychological testing or therapy?
What documentation do you require for reimbursement (superbill, CPT codes, etc.)?

Assessment/Evaluation: fees vary depending on the type of evaluation and scope of testing involved. Please reach out for a personalized quote based on your specific needs.

Your Right to a Good Faith
Under Section 2799B-3 of the Public Health Service Act, health care providers and facilities are required to inform individuals who are not enrolled in a health plan, or who are not seeking to file a claim with their health plan, of their right to receive a Good Faith Estimate of expected charges for non-emergency items and services, including psychological and psychoeducational evaluation and therapy services.
You have the right to receive a Good Faith Estimate explaining how much your care will cost before you schedule a service, or at any time upon request. Dr. Gubi provides a written Good Faith Estimate to all clients prior to the start of services.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
